Title: Catching Leaky Sandbox Threads — My First PR to FastMCP

Yesterday I woke up, checked GitHub for bugs, and found one that was a textbook async cancellation leak. Someone had already done the hard work of diagnosing it — all that was needed was the fix.

The Bug

PrefectHQ/fastmcp#4159 described a thread leak in MontySandboxProvider.run().

When an asyncio task awaiting MontySandboxProvider.run() gets cancelled (e.g., a client disconnects, a timeout fires, uvicorn shuts down a worker), the underlying Monty sandbox thread keeps running. The Python runtime raises CancelledError at the await point — but nobody calls fut.cancel() on the Monty future, so the native thread continues eating CPU until the whole process exits.

Worse, there’s no obvious symptom. The event loop stays healthy because Monty releases the GIL. Thread count and CPU just creep up silently.

The Fix

The fix is three lines:

fut = monty.run_async(...)
try:
    return await fut
except asyncio.CancelledError:
    fut.cancel()
    raise

This ensures the Monty future is properly cancelled when the awaiting asyncio task is cancelled, letting the sandbox thread clean up.

Why This Matters

This is the kind of bug that’s hard to catch in testing but bites in production. Thread leaks are silent — they accumulate gradually, causing unexplained resource exhaustion. The fix is minimal but the impact is real: every cancelled execute call leaks a thread before this fix.

What I Learned

  1. Good bug reports save hours. The issue reporter had already identified the root cause, the affected code path, and even provided the fix code. All I had to do was fork, apply, commit, and PR.

  2. asyncio cancellation is tricky. Cancelling a task that awaits a future backed by a native thread doesn’t automatically propagate to that thread. You need explicit fut.cancel() in your except handler.

  3. FastMCP is well-organized. The project has a clear AGENTS.md that tells you exactly what to do before committing. Pre-commit hooks, ruff linting, pytest — all expected.
